Ps2 controller config

can someone cut and paste their controller config? I am using a ps2 controller with the radioshack adapter. I downloaded the one that is on the site but for some reason my cnonfiguration is not right when I throw to first base it goes home. This is my config:

Here is how I have mine set up. I know the shape buttons are correct, but I had to improvise with what to assign the L2, R2, L3, R3 buttons to. I think this is a pretty good set-up.
