Just needs a quick couple fixes (nose, little work on the mouth, and blending), and it should be good to go. I OEdited him perfectly to remove the long chin, but I guess you can't edit 04 faces without making the face crash in the game, so I ended up not being able to OEdit it. I'm trying to think of an 05 headshape which looks similar... I believe someone did a Gabe Kapler face which is an 05 face, since both Kouzmanoff and Kapler have sort of a 'muscular' face.
