PS2/USB - Pitch 5 Problem

I've tried searching for, like, an hour, but I couldn't find posts that would solve this.

For some reason, my Pitch 5 will occasionally not work, no matter what button it's mapped onto. It stops working for about 5 minutes at a time, then comes back. At first, I just assumed my USB adapter was acting up and the key it's mapped to (R2) wasn't being detected properly. But when I switched it out with other pitches, the problem follows Pitch 5. For example, Pitch 2 might work on Circle, but if I swap the keys, Pitch 5 won't work on Circle, yet Pitch 2 will work on R2.

To make sure that it wasn't still the controller, I then mapped Pitch 5 onto the keyboard and it still wouldn't work. When I select a standard keyboard configuration, though, it will work just fine.

Again, Pitch 5 works about 80% of the time, but when it stops, it won't come back for a few innings. I've been trying to find some sort of pattern to it, but it seems completely random. So I now throw myself upon the merciful feet of The Internets. Does anybody know how to fix this? I saw a mention of this problem for a Logitech gamepad here:
